Health Information Forms
These forms need to be completed before a therapist or physician can be consulted at any of the Center for Sports Medicine clinics. You can inspect them, or you may print, complete and bring them in to save time when you register. However, we do have to go over the forms and discuss them with you, even if you have already filled them out.
