At Saint Francis Memorial Hospital, social workers, nurses and community organizations work together to arrange appropriate post-discharge care for each individual. After leaving the hospital, you or your loved one may need medical equipment, home care or hospice care.

Case managers work with patients and their physicians to navigate their insurance, ensuring the best possible aftercare. Case managers also link uninsured patients to free clinics and services.  Saint Francis' partnership with community organizations such as Glide Memorial, St. Anthony's Foundation, Friends of the Elderly and other resources directly benefit those who may need financial help.
